Sudden testicle pain: why waiting for it to pass is risky

Testicle pain can have several causes, including infection, injury or a twisted testicle. When a testicle twists, its blood supply can be cut off. The NHS says sudden, severe testicle pain needs immediate hospital care; pain with nausea, vomiting or abdominal pain, and pain that lasts more than an hour or continues at rest, also require it. You cannot reliably distinguish these causes from the pain alone.

This is not advice to wait one hour. If pain is sudden and severe, go now. Tell the person arranging transport or the emergency department exactly when it began, which side hurts and whether you have swelling, nausea or vomiting. Do not delay the trip to complete a questionnaire or search for a perfect description.

A twisted testicle may need urgent surgery to restore blood flow. The chance of permanent damage rises with delay, so an online guide cannot safely decide whether it is torsion. It can happen beyond the teenage years, and it does not always follow an injury. A clinician must assess you in person.

Sometimes a sudden episode eases because the testicle untwists. Healthdirect warns that this still needs medical assessment as soon as possible because the problem can recur. Feeling better is not proof that the episode was harmless. If severe pain is happening now, use the nearest emergency service; if a sudden episode has stopped, seek prompt in-person assessment and explain what happened.
